Pathways Forensic & Mental Health Intake Forms
Our goal is to spend as much time with you or your child as possible during your initial intake session at Pathways FMHS. If you have an appointment scheduled, completing certain mental health intake forms prior to your appointment will save a significant amount of time.
For your convenience, you may download and print the adult intake and biopsychosocial (BPS) forms to complete, then bring with you to your appointment. If you have scheduled counseling for one or more children, please complete an adolescent intake form and the Child Behavior Checklist for each child.
